The property located at 5301 West Madison is a church which has been converted into an Adult day Care Program. They are performing construction/remodeling of a major kind without building permits. They have knocked down two interior walls while patients have been in the building exposing them to dust and unknown airborne pollutants. This is a major health hazard. As of this date they have added three interior rooms, a new kitchen, and a smoking room all without permits. Also, the building is not zoned for the type of services being performed in the building. I will be calling the ombudsman over Illinois nursing homes on Monday. I thought that before I alerted this agency the city should, in fairness, be told of the infractions.
